Our mission is to provide choices, resources and services to older persons and to persons with disabilities to enhance independence, dignity and self-worth.
Services and Programs:
Aging and Disability Resource Center (ADRC) - The ADRC serves the general public as a source of information and assistance regarding long term care resources for persons who are elderly or disabled and their familys. Link to ADRC web.
Alzheimer Family Caregiver Support Program (AFCSP) - AFCSP provides respite and support to family members and friends who are caregivers for people with Alzheimer's Disease and other related dementias.
Community Options Program (COP) - COP is designed for elderly or disabled persons who have a long-term disability or illness and who wish to remain living at home. Protective Services - How to Report Link to definitions
Supportive Home Care Program (SHC) - The SHC Program funds assistance to elderly or disabled persons who are in need of services but do not meet the COP eligibility criteria. This program is also designed to help people remain independent in their own homes.
