    Age 16 and over - $100.00
Valid for ten years

Underage Age 16 - $85.00
Valid for five years

Renewal - $75.00

FEES:  PASSPORT CARD

Age 16 and over   - $45.00

Age 15 and under - $35.00

Passport Application Form

Two 2x2 Photographs

    Certified copy of Birth Certificate

    Prior passports must be submitted

    Driver's License or Wisconsin Photo ID

NOTE:

    Passport applicant under the age 16 must appear with
both parents to sign the application.

Applicants between the ages of 16 and 17 must appear with
one parent to co-sign the application. 

EXPEDITED SERVICE

Those wishing to receive passports sooner may request expedited service and two-way overnight mail to receive passports within 2-3 weeks from the date of application.  (Additional cost per application is $60.00, plus two-way overnight mail fee.)  For those with proof of more urgent departure, in-person application at a Passport Agency is suggested.
