Family Care is a long-term care service system with goals to support people's independence, choice and access to services and supports to meet their needs, and improve overall quality of life through a cost-effective long-term care system. Western Wisconsin Cares (WWC) operates the Family Care Program serving the counties of Trempealeau, Buffalo, Clark, Jackson, La Crosse, Monroe, Pepin, and Vernon. IRIS is a long-term care program where people are able to self-direct their publicly funded, community-based, long-term care supports and services. The FCSP provides information and assistance to family members and friends who help or care for persons age 60 and older and/or grandparents or relative caregivers age 55 and older who are caring for children under age 19 or who are caring for a relative with a disability. The AFCSP provides information and assistance to family members and friends who help or care for individuals with Alzheimer’s disease and related disorders regardless of age.
